Maryland's Department of Health is re-opening its deadline for applications.

The Maryland Department of Health is excited to share that the State is continuing to accept applications for AHEAD Medicaid Path.

The Maryland Department of Health (MDH) will begin accepting applications on January 1, 2026. Non-MDPCP organizations can apply by completing the State Vetting Application no later than 5:00pm this Friday, July 11.

To be eligible for the Medicaid Path, practice TIN organizations must:

  • Have at least 250 assigned HealthChoice members
  • Be connected to CRISP or plan to be connected to CRISP by January 1, 2026
  • Be able to report electronic clinical quality measures (eCQMs)

Eligible organizations that meet the vetting criteria above will be contacted by their partner MCOs to sign a contract addendum to begin participating in the Medicaid Path January 1, 2026.

Organizations that choose to participate in the Medicaid Path and are not currently participating in the Maryland Primary Care Program (MDPCP - Medicare Path 2) may also indicate their interest in participating in PC AHEAD (Medicare Path 1). The Centers for Medicare and Medicaid Services (CMS) will share more information on PC AHEAD in the near future. Visit the MDH Primary Care Program website for more information including the Medicaid Path.
