The Warm Handoff: Linking Primary Care, Patients, and Local Health Departments

Join MDAFP and MACHO on Tuesday, January 20 at 7:00pm for a live, virtual, webinar session to learn more about making the link between primary care and public health. 

Time with your patients is precious. Physicians must collect histories and assessments; the patient questions and concerns need to be addressed; and follow up instructions must be conveyed clearly and effectively. This session is meant to support physicians by educating them on what's available in local communities that support your patient’s health and well-being and how to make that warm hand off. 

This virtual webinar will be presented by Maryland Health Officers from the Maryland Association of County Health Officers (MACHO – may-CHO) who will make the connection to local health departments and the services and programs they provide in ways that maximize patient care and shared outcomes. Officers will also provide an opportunity for attendees to provide feedback and engage in discussion about how to keep lines of communication open.   1. Know where to find the contact information for the local health department(s) in their catchment area
  2. Describe the Local Health Officer’s Chief Health Strategist role in and for the community
  3. Develop ways to identify resources in your community of relevance to your patients
  4. Formulate a plan on how to collaborate and coordinate with the local health department(s) in their catchment area
  5. Consider ways to get involved in your local community
  6. Examine how to connect and integrate primary care and public health to achieve shared health outcomes
  7. Provide feedback to Local Health Officers about primary care, patient referrals, and areas of potential collaboration
